Who was the ghost?
Gwen
How did s/he die?
Killed in an accident.
What was their unfinished business?
Gwen and Her mother got into a car crash right after She deleted a chain letter and now is trying to continue the chain letter in hopes that Her mother wakes from coma since She thinks the chain letter is the reason the car crash happened. Melinda tells Gwen that it wasn't the chain letter and that Her mother had cancer. Gwen and Her mother then go into the light.

"See No Evil" is also an NCIS episode... LOL. My 2 favorite shows are NCIS & Ghost Whisperer, right? Well, "Leap Of Faith" AND "See No Evil" are NCIS AND Ghost Whisperer episode titles! Doesn't it work really well to be in love with NCIS & GW?! LOL♥
